I am in-network with Blue Cross/Blue Shield PPO only. Your individual plan will determine the cost of each visit, and can be determined by contacting Blue Cross/Blue Shield directly.
If you have any other type of insurance, I can provide you with an itemized bill to submit in the event that you are entitled to out-of-network benefits. You may obtain this information by calling your insurance carrier to inquire.
The out of pocket cost per session in $165. Payment is required at the time of service and may be rendered with cash, check, or credit/debit card.
